TK/K parents: Kindergartners need to have the Physical Exam form (CDHP form) and the Oral Health form completed by their doctor/dentist and on file in the school office before the start of first grade. Sixth grade Parents/Guardians: All students going into 7th grade must have proof of a Tdap of a Tdap booster (tetanus, diphtheria, and pertussis) prior to the start of the 2024-2025 school year. Please submit records no later than June 7, 2024. Students without a record of a Tdap booster will not be able to receive their fall schedule until this is completed.
