What shots are required for school entry in California? Find out here!
The Physical Exam form (CDHP form) and the Oral Health form must be completed for kindergarteners by their doctor/dentist and on file in the school office before the start of first grade. Email to [email protected] or submit a paper copy to the front office.
​​If your child needs medication during the school day, we must have this form completed by a parent / guardian and the prescribing physician.

Health Center Resources:

School based health center services are available to students till they are 22 years of age. Completion of consent form required for services. To have medical or dental consent at the health center, please fill out the following forms:

Medical Consent-- Please fill out the medical consent form if you would like your child to have the ability to see our health care provider or participate in health education while at school.

Dental Consent-- Please fill out the dental consent form if you would like your student to receive an oral health assessment or dental services including x-rays, cleanings, and restorative treatments including fillings, sealants, extractions, etc.
